A spermatocele is a benign cystic swelling that forms near the epididymis and contains a milky fluid rich in sperm. It is typically painless and presents as a small, soft lump in the scrotum; often discovered incidentally. Although noncancerous, it may be evaluated if there is uncertainty about structure, pain, or growth. Most cases are asymptomatic and require no treatment.
